THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Meadows Place Associates, LLC is required by law to maintain the privacy and security of your protected health information (PHI), to provide you with this Notice of our legal duties and privacy practices, and to follow the terms of this Notice currently in effect. We are also required by law to notify you following a breach of your unsecured PHI.
We may use and disclose your protected health information without your authorization for the following purposes:
We may use and disclose your PHI to provide, coordinate, or manage your healthcare and related services. This includes sharing information with other healthcare providers involved in your care.
We may use and disclose your PHI to bill and collect payment for healthcare services provided to you. This may include disclosures to billing services or payment processors.
We may use and disclose your PHI for healthcare operations, including internal administration, quality improvement activities, care coordination, training, auditing, licensing, and other activities necessary to operate our practice.
We may disclose your PHI without your authorization as permitted or required by law, including disclosures for public health activities, health oversight activities, legal proceedings, law enforcement purposes, workers’ compensation, compliance with laws and regulations, or to prevent or lessen a serious and imminent threat to health or safety.
We will make reasonable efforts to limit the use or disclosure of PHI to the minimum necessary to accomplish the intended purpose, except where disclosure is required by law or permitted for treatment purposes.
We will not use or disclose your PHI for purposes other than those described in this Notice unless you authorize us to do so in writing. Written authorization is required for most uses of PHI for marketing purposes, the sale of PHI, and most disclosures of psychotherapy notes, if applicable. You may revoke your authorization in writing at any time, except to the extent that action has already been taken in reliance on your authorization.
Meadows Place Associates, LLC may communicate with you electronically, including through telemedicine platforms, patient portals, email, or text messaging, when permitted by law and consistent with reasonable safeguards. While we use HIPAA-compliant systems to protect your information, electronic communications carry inherent risks. By receiving services, you acknowledge and accept these risks.
You have the right to inspect and obtain a copy of your medical records and certain other health information we maintain about you. Requests must be submitted in writing and will be processed within the timeframes permitted by law.
You have the right to request an amendment to your health information if you believe it is incorrect or incomplete.
You have the right to request restrictions on certain uses or disclosures of your PHI. We are not required to agree to all requested restrictions.
You have the right to request confidential communications, including receiving communications at an alternative address or by alternative means.
You have the right to request an accounting of certain disclosures of your PHI.
You have the right to receive a paper copy of this Notice at any time, even if you have agreed to receive this Notice electronically.
You have the right to be notified in the event of a breach of your unsecured PHI.
We reserve the right to change this Notice at any time. Any changes will apply to all PHI we maintain. The revised Notice will be available upon request and will be posted on our website.
If you believe your privacy rights have been violated, you may file a complaint with Meadows Place Associates, LLC or with the U.S. Department of Health and Human Services. You will not be penalized or retaliated against for filing a complaint.
